Have you been experiencing a chronic injury? Are you suffering from a Chronic disease or illness? If so, you may be eligible for a CDMP through your GP. Consequently, this CDMP allows you to access 5 sessions in a calendar year with an allied health professional. Medicare also subsidizes this.

What is a chronic condition?

A chronic condition is defined as an injury, disease or illness that has affected you for over 6 months. This process is completely directed by a GP. During this, they take into account your current situation and previous medical history.
