DVA

Longevity Exercise Physiology provides services to those covered under the Department of Veteran Affairs (DVA). Longevity’s Exercise Physiologists manage chronic health conditions and/or injuries by providing exercise rehabilitation for Veterans to meet their clinical needs. These services intend to give Veterans the skills to effectively self-manage their health conditions through exercise.

Additionally, Exercise Physiology can help manage health conditions including:

  • Diabetes
  • Arthritis
  • Cardiovascular disease
  • Osteoporosis
  • Cancer
  • Chronic musculoskeletal conditions

To be eligible for DVA services, veterans must hold a DVA white card or DVA gold card. Veterans also need to be referred, via a D904 form, to an Accredited Exercise Physiologist for clinically necessary treatment. However, you can be referred by a General Practitioner (GP), Medical Specialist or Health Professional as part of a hospital discharge.

White cardholders are eligible to receive treatment for specific conditions or injuries caused by an accepted war or service.

Gold cardholders are eligible to receive treatment to meet their clinical needs for all their conditions. Also they can receive treatment if they are classed as totally or permanently incapacitated (TPI).

A referral is valid for up to 12 treatment sessions or one year, whichever ends first (except for TPI Gold Card). However, your referring practitioner will issue a new referral if it is required.
